Abstract

           The elderly have limited regenerative abilities and are faced with numerous physical, psychological and social role changes that challenge their sense of self and capacity to live happily. This action research aimed to develop a health care model for elderly by community participation, Nonglake Subdistrict, Kosum Phisai District, Mahasarakham Province. The research participants were divided into two groups. The first group was elderly with dependent total of 74 people. The second group was stakeholders of elderly health care by selecting specific of 54 people. Four steps in the development of the model were: planning, acting, observing, and reflecting. The research was carried out from January 2017 to May 2017. The data were analysed using descriptive statistics, t-test, and content analysis.

           The results revealed thati1)ithe health care model for elderly by community participation included: (1)iestablishing a multidisciplinary teamwork networks (2) enhancing a potential of the teamwork networks (3) establishing an ElderlytSchoolt(4)trainingt for tcaregivers (5) home visiting by caregivers (6)supervision by preceptors team (7) knowledge management and giving back the data to community 2) the scores of elderly’ quality of life after implementing the model were higher than at the baseline (p < .05). The key success factors of the development included 1) the strengthening of members committee at sub-district level 2) the mutual cooperation of multidisciplinary teamwork and 3) the dedication of care of caregivers.
