Thailand has a well-organized and appropriate public health service system in order to ensure good health for the people. But various changes make the health system that is dynamic has become more complicated Researcher therefore has an idea to review, study and analyze the situation and trends of the Thai health system. This study is a descriptive study. To review the academic documents of the health system, evolution, philosophy and related elements from past, present to trend of Thai desirable health systems the future. The objectives of this study were to study, analyze the situation of health system and the management of Thai health systems. In order to develop policy proposals and directions and trends in the management of health systems in Thailand at present and in the future for health security in the medium and long term period. The study indicated that there are 3 major health insurance systems in Thailand: 1. Civil servant welfare system, 2. Social security system and 3. Universal health insurance system. The public health insurance thus everyone is covered with one of the 3 types of collateral. By providing comprehensive health services in all dimensions including medical treatment, health promotion disease control prevention and rehabilitation. With both private health services and government facilities. The service facilities available at all levels, there is a primary service system as part of the service system under the supervision of two main departments, namely the Ministry of Public Health and the National Health Security Office. The overall of the Thai health insurance system found that those who played an important role in the Thai health insurance system consisted of 3 main sectors, namely service buyers, service providers, and people who were entitled to receive services according to their rights. From the results of the study, it is suggested that the management must be balanced and the highest satisfaction of all stakeholders. There should be a national central agency. That can take care of the overall health system of the country, Thai health insurance system in the future. Should be a source of funds from the tax and management of health service systems. The Ministry of Public Health needs to review the role of the agency in order to support changes in various contexts and be able to continue to work effectively.
