This quasi-experimental study using one group pretest-posttest design was conducted to study the effects of the nursing discharge plan program on knowledge and goal attainment in caring for stroke patients.
A sample of 30 patients were diagnosed with stroke and admitted in Phachi hospital from 1st July 2019 to 31st December 2019. These samples participated in the 2-weeks program consisted of (1) background knowledge assessment and outcome expectations after treatment, (2) providing specific education along with METHOD protocol, (3) goal setting among the patients, caregivers and nurses, and (4) monitoring on goal attainment after two weeks of discharge. The instruments for data collection included the knowledge test and instructional interview. The data were analyzed using frequency, percentage, mean, standard deviation, and paired t-test.
The results revealed that 1) the mean scores of knowledge of the patients after receiving the nursing discharge plan program was significantly higher than before receiving the program at the significance level of .05 (t= 5.711, p= .0001) 2) after two weeks of discharge, 90% of the sample achieved their goal attainment. Considering the aspects of outcomes, 15 cases (50%) met on the self-care aspects for taking medication, diet control, and daily activity. Meanwhile, 12 cases (40%) met on receiving continuous treatment in terms of follow-up and rehabilitation regularly. There were only 3 cases (10%) that could not meet their goal attainment on the aspect of daily activity.
