Evaluation of Effectiveness of the Centralized Medication Error Recording Program in Pharmacy Department, Chonburi hospital

Abstract
The purposes of this study were to 1) compare the effectiveness of a new medication error recording program developing since February 2017 to May 2017 with the original program since October 2016 to January 2017, 2) find trend of data recording and 3) analyze data from the new medication recording program. The research methodology involved 2 parts; medication error results from 2 programs and 32 Pharmacists satisfaction from questionnaires. The questionnaires consist of 3 parts; general information, satisfaction question and recommendations. The satisfaction questions were validated by experts. The data were analyzed by descriptive statistics; mean (µ), standard deviation (σ), frequency and percentage. The new program could more detect medication errors than the original one. The detected medication errors were increased from 734 to 2,579 events. The data recording trend was 3.5 fold increased. The pharmacists’ satisfactions is very great with the new program.
Conclusion; the new medication error recording program is more effective than the original one.
