A Study of Thai Traditional Medicine Services Rate in Public Healthcare Facilities under the Ministry of Public Health

บทคัดย่อ

การศึกษาอัตราค่าบริการด้านการแพทย์แผนไทยของสถานบริการสาธารณสุขของรัฐกระทรวงสาธารณสุขเป็นการศึกษาเชิงสำรวจ วิเคราะห์ต้นทุนต่อหน่วยกิจกรรมบริการการรักษาและฟื้นฟูสภาพด้านการแพทย์แผนไทย เก็บข้อมูลปีงบประมาณ พ.ศ. 2564 - 2565 แบ่งเป็น 2 ขั้นตอน ขั้นตอนที่ 1 สำรวจข้อมูลการจัดบริการการแพทย์แผนไทยและอัตราค่าบริการของหน่วยบริการสาธารณสุขของรัฐทุกแห่งในส่วนภูมิภาค สังกัดกระทรวงสาธารณสุข ด้วยการใช้แบบสำรวจสถานการณ์การจัดบริการการแพทย์แผนไทยและอัตราค่าบริการรายกิจกรรมจากแผนกการแพทย์แผนไทยของหน่วยบริการ มีหน่วยบริการตอบแบบสำรวจ จำนวน 2,435 แห่ง ประกอบด้วย โรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) จำนวน 1,871 โรงพยาบาลชุมชน (รพช.) จ????ำนวน 472 แห่ง โรงพยาบาลทั่วไป (รพท.) จำนวน 67 แห่ง และโรงพยาบาลศูนย์ (รพศ.) จำนวน 25 แห่ง วิเคราะห์และจัดการข้อมูลในเบื้องต้น จากนั้นคัดเลือกรายการกิจกรรมบริการที่จะนำมาจัดทำอัตราค่าบริการตามเกณฑ์ที่กำหนด พร้อมจัดทำแนวทางการจัดบริการรายกิจกรรม ขั้นตอนที่ 2 เก็บข้อมูลต้นทุนอัตราค่าบริการรายกิจกรรม ด้วยการคัดเลือกกลุ่มตัวอย่างแบบเฉพาะเจาะจง (Purposive Sampling) ที่เป็นตัวแทนของประชากรที่ศึกษาจากขั้นตอนที่ 1 ที่มีการจัดบริการแพทย์แผนไทย ที่ครอบคลุมรายการกิจกรรมบริการ แบ่งเป็นระดับ รพศ./ รพท. เขตสุขภาพละ 1 แห่ง รวม 12 แห่ง ระดับ รพช. เขตสุขภาพละ 3 แห่ง รวม 36 แห่ง และระดับ รพ.สต. เขตสุขภาพละ 2 แห่ง รวม 24 แห่ง รวมทั้งสิ้น 72 แห่ง โดยใช้แบบสำรวจข้อมูลต้นทุนรายกิจกรรมบริการการแพทย์แผนไทยของหน่วยบริการในสังกัดกระทรวงสาธารณสุขโดยมีรายละเอียดเกี่ยวกับข้อมูลต้นทุนทางตรง ประกอบด้วย ข้อมูลค่าแรง ข้อมูลค่าวัสดุสิ้นเปลือง และข้อมูลค่าลงทุนโดยใช้ค่าสถิติเชิงพรรณนา ได้แก่ จำนวน ร้อยละ และค่ามัธยฐาน

ผลการศึกษาอัตราค่าบริการด้านการแพทย์แผนไทย จากการวิเคราะห์ต้นทุนต่อหน่วยกิจกรรมบริการการรักษาและฟื้นฟูสภาพด้านการแพทย์แผนไทย จำนวน 16 กิจกรรมบริการ ดังนี้ 1) เวชกรรมไทย 199.08 บาท 2) การนวดแผนไทย 379.08 บาท 3) การประคบสมุนไพร 295.59 บาท 4) การอบไอน้ำสมุนไพร 293.58 บาท 5) การทับหม้อเกลือ 375.40 บาท 6) การนวดแผนไทยและประคบสมุนไพร 469.08 บาท 7) การพอกเข่า 259.16 บาท 8) การพอกท้อง 260.00 บาท 9) การพอกตา 195.88 บาท 10) การรมยาสมุนไพร 213.14 บาท 11) การแช่ยาสมุนไพร 250.23 บาท 12) การอาบยาสมุนไพร 287.39 บาท 13) การเผายาสมุนไพร 276.57 บาท 14) การนั่งถ่าน 233.24 บาท 15) การบ่งต้อด้วยหนามหวาย 190.74 บาท และ 16) ชุดกิจกรรมบริการฟื้นฟูมารดาหลังคลอด 909.18 บาท

การศึกษาอัตราค่าบริการด้านการแพทย์แผนไทยของสถานบริการสาธารณสุขของรัฐกระทรวงสาธารณสุข ทำให้ทราบถึงต้นทุนการให้บริการของแต่ละกิจกรรม สามารถนำข้อมูลไปพิจารณากำหนดอัตราค่าบริการการบริหารต้นทุน

Study of Thai Traditional Medicine Service Rate in Public Health Facilities under the Ministry of Public Health. The study was analyzing the unit cost per service activity for TTM treatment and rehabilitation. Data were collected for the fiscal years 2021–2022. The study was conducted in two phases as follows: Phase 1: This phase involved surveying the status of TTM services and activity-based service rate across all regional public health facilities under the Ministry of Public Health. Data were collected using a survey regarding TTM service management and specific activity fees from TTM departments. A total of 2,435 facilities responded, including subdistrict health promoting hospitals 1,871 units, community (district) 472 units, general hospital 67 units and regional hospital 25 units. The preliminary data were analyzed to select service activities for rate setting based on established criteria, alongside the development of guidelines for activity-based service management. Phase 2: Collection of activity-based cost data a purposive sampling method was used to select representative facilities from phase 1 that provide comprehensive TTM services. The sample included 72 facilities in total, categorized as follows: 12 Regional/General Hospitals (1 per Health Region), 36 Community Hospitals (3 per Health Region)
and 24 Health Promoting Hospitals (2 per Health Region) Data were collected using a TTM activity-based cost survey specifically designed for units under the Ministry of Public Health. The details focused on direct costs, which include labor costs, material costs and capital costs. Utilizing descriptive statistics, such as frequencies, percentages, and the median.

The study analyzed the unit cost of 16 Thai Traditional Medicine (TTM) treatment and rehabilitation service activities. The resulting median unit costs were calculated as follows: 1) Thai traditional medicine practice 199.08 THB 2) Thai massage 379.08 THB 3) Herbal compress 295.59 THB 4) Herbal steam 293.58 THB 5) Hot salt pot compression 375.40 THB 6) Thai herbal compress massage 469.08 THB 7) Herbal poultice 259.16 THB 8) Abdomen herbal poultice 260.00 THB 9) Eye herbal poultice 195.88 THB 10) Herbal moxibustion 213.14 THB 11) Herbal soak 250.23 THB 12) Herbal bath 287.39 THB 13) Herbal burning 276.57 THB 14) Charcoal sitting (Nung Tarn) 233.24 THB 15) Bong-Tor with rattan thorn 190.74 THB and 16) Postpartum recovery service package 909.18 THB.

This study on TTM service rates in public health facilities provides crucial data on the service provision cost for each activity. The findings can be utilized for determining standardized service rates, improving cost management and enhancing the efficiency of service delivery and resource allocation within public health units.
