Outcomes of Pre-operative Preparation for Patients before Receiving Anesthesia for Total Knee Arthroplasty Mahasarakham Hospital

Abstract

ABSTRACT

           This action research aimed to develop guidelines of pre-operative preparation for patients before receiving Anesthesia for total knee Arthroplasty, and study the outcomes of the guidelines of pre-preparation for patient before receiving anesthesia for knee arthroplasty at Mahasarakham Hospital. The research was conducted from August 2019 to September 2020. The participants of this research included: 10 nurse anesthetists, 10 professional nurses and 63 knee prosthetic patients undergoing surgery. An instrument used in conducting this research was brochure and video clip regarding “The performance before anesthesia for surgery and the performance after surgery”. The instruments applied to collect research data included:
pre-post anesthesia visit form, patient assessment form before surgery ASA Class, Anesthesia record form, assessment form of patient satisfaction and anxiety. The obtained data was analyzed percentage, means, standard deviation paired t-test and content analysis.

           The results of this research demonstrated that the guidelines of pre-operative preparation for patients before receiving Anesthesia for total knee Arthroplasty at Mahasarakham Hospital consisted of 1) the assessment of readiness and anxiety for care planning, 2) set criteria and evaluate patients’ readiness before receiving Anesthesia for operation, 3) creating educational media, brochure and video clip regarding “The performance before anesthesia for surgery and the performance after surgery” for the patients and their relatives, 4) planning for anesthesia, prevention of unfavorable cause/death, 5) forwarding information of preparing for the surgery day within the nurse anesthetist team, 6) follow-up after surgery for nursing plan. The results of the outcome of pre-operative preparation for patients before receiving anesthesia revealed that the average anxiety of the patients was reduced in all aspects, and their satisfaction was at high to highest level.In terms of complications, it was found that the patients had hypotension 25.40%, shivering 20.63%, arrythmia 14.29%, nausea/vomiting 4.76%, sore throat 3.17%, back pain 3.17%, pain in PACU 1.59%, itching 1.59%.
The patients with such complications got proper problem management/ care from the employment of the guidelines. The results indicated that the
pre-operative for patients before receiving anesthesia for total knee arthroplasty could be able to reduce the incidence of cardiac complications and blood flow after receiving anesthesia. No postponement incidence was found due to physical and mental instability.
