The outcomes of Diabetic Patients care During the Prevention of Covid-19 Pandemic Situation at Wapi Pathum Hospital Maha Sarakham Province

          From the situation of the epidemic of Covid-19 in 2021, Wapi Pathum Hospital There has been a project to send medicines home to some chronic disease patients voluntarily. To reduce congestion and prevent the epidemic of Covid-19 according to the policy "Stay at home, stop germs, for the nation"   It has started to be delivered at home by a team of family doctors. and home health volunteers from October 1, 2020. In 2021, medicines will be delivered to patients at home 1-2 times per appointment, which does not require a visit to the doctor for approximately 3-6 months. Objective: To study the outcome of diabetes treatment in the situation to prevent the spread of COVID-19. At Wapi Pathum Hospital and to compare the treatment outcomes for diabetes patients between those who received continuous treatment at the hospital and those who delivered medicines to their homes. This research is an analytical research. Collecting blood glucose (HbA1c, FBS, LDL, eGFR) data from laboratory results. From Electronic Database (HOSxP) of Wapi Pathum Hospital Comparison between 2020 and 2021. The sample consisted of 775 patients with diabetes who received continuous treatment at Wapi Pathum Hospital and the group who voluntarily delivered medicines to their homes. Data were analyzed by descriptive statistics. and analytical statistics comparing mean HbA1c, FBS, LDL, eGFR in 2020 and 2021 with Pair t-test statistics and comparing the mean between groups by Paired t-test at a significance level of 0.05. The results showed that most of them were female, 61%, mean age 61.5±10.0 years, mean illness duration 7.8±3.1, had a history of other co-morbidities 61.4%. Treatment outcomes for diabetes patients in 2020 found that HbA1C <7% 25.8, FBS <130 mg/dl, 28.4%, LDL<100 mg/dl, 32.3%, eGFR ≥90 ml/min/1.73m2, 56.3 %, and in 2021, it was found that HbA1C <7%, 22.7%, FBS <130mg/dl. 31.9%, LDL<100 mg/dl 34.3%, eGFR ≥90 ml/min/1.73m2 58.1% Comparison of treatment outcomes for diabetes patients between the group who continued treatment at Wapi Pathum Hospital and the group who delivered the medication home. In the COVID-19 situation, it was found that the mean HbA1C = 8.23 ±2.13 and 8.36±1.95 (P-value =.880), FBS = 165.2±56.1 and 159.39±53.2 (P-value = .015), LDL=107.64. ±31.45 and 106.19±33.13(P-value=.453), eGFR =89.86±18.91 and eGFR =90.04±19.51 (P-value =.280). There’re no statistically significance in HbA1C,LDL and eGFR except in FBS level.Drug delivery maybe a good choice for Diabetic service to reduce the cost and visiting of patient in Diabetic clinic.
