Development of Stroke Patients Care Model

Abstract

This research and development aimed to develop and study the outcomes of a stroke patient care model Wapi Pathum Hospital Wapi Pathum District Maha Sarakham Province. The research was conducted in 4 phases: 1) situation study, 2) model development, 3) trial and 4) evaluation. The sample consisted of 3 groups: 83 stroke patients, 83 primary caregivers, and 36 multidisciplinary tools. 1) Patient Care Guide 2) Bathel Index Score Assessment Form 3) Care Plan 4) caregiver knowledge test;
5) caregiver competency assessment form; 6) caregiver satisfaction questionnaire; 7) care plan compliance check form; 8) multidisciplinary team and caregiver interview form. Data were collected from October 2017 to March 2019. Data was analyzed by frequency distribution, percentage, mean, comparison of mean differences with Paired t-test. Qualitative data were used for content analysis.

            Research Result Wapi Pathum Hospital Network 2. Outcomes on patients showed a statistically significant reduction in violence (p = <.001), an increase in ability to perform daily activities (p = <.001), a decrease in disability (p = <.001) bedtime was decreased (p = .015) and medical expenses were reduced (p = .035). Respiratory tract infections and pressure sores were not detected. urinary tract infection and recurrence within 28 days was 1.6%. It was found that most multidisciplinary practices followed the guideline 100%, nutrition practiced 40% and overall satisfaction was at a high level. Therefore, the model should be applied so that stroke patients are cared for by their caregivers. and multidisciplinary care together continuously, resulting in better treatment results.
