Nursing Care of Patients with Heart Failure : Case study
Abstract
ABSTRACT
Objective : To study the nursing care of patients with heart failure in clinic in Out Patient Department.
Methods : A comparative study of 2 patients admitted to Trang Hospital. The study was conducted between September 1, 2023 and December 31, 2023. The tools used are interviews, observations, and medical records.
Results : It was found that the 2 patients had similarities and differences in various factors as follows:
Diagnosis It was found that both patients had risk factors for congestive heart failure. The first patient has a history of Atrial fibrillation and Mitral Stenosis. The second patient had a vascular dysfunction, which LV Thrombosis was found.
Signs and symptoms Patient 1 came with chest pain, heart palpitations, got easily tired when expending energy, functional class II. Echocardiogram examination found severe MS, LVEF 14%, The 2nd patient came with fatigue, unable to lie down, swollen feet, abdominal pain, chest pain, palpitations, functional class III-IV. Echocardiogram examination found severely impaired LV systolic function, LV thrombus LVEF 30%. Patients had different treatments, as it was found that patient 1 had undergone heart valve replacement surgery to solve problems that cause disease, while the patient 2 refused treatment by coronary angiography. Patients had different perceptions and self-care.
Conclusion : The role of nurse is therefore important in evaluating the patient's problems before and after seeing a doctor. The nurse must have knowledge and understanding the pathology of the disease. Knowledge of self-care and disease-specific symptom changes of heart failure patients can be assessed. The nurse must advice to patients on how to take care of themselves to prevent, treat and control diseases that are risk factors, or the cause of heart dysfunction leading to heart failure.
