Development of the care model for sepsis patients using the fast-track service system, Kae Dam Hospital, Kae Dam District, Maha Sarakham Province

Abstract
The objectives of this research and development were to develop and study the outcomes of the care model for patients with sepsis using the fast-track service system at Kae Dam Hospital in Kae Dam District, Maha Sarakham Province. The study sample included : 1) personnel, doctors, pharmacists, nurses, infection prevention and control nurses and medical technicians totaling 12 individuals, and 2) patients with sepsis, selected through purposive sampling, totaling 51 individuals. The study was conducted in three phases: preparation, development and implementation of the care model, and outcome evaluation. Research tools included manuals and models for caring for patients with sepsis. Data collection tools: 1) knowledge assessment form, 2) system compliance assessment form, and 3) satisfaction questionnaire. The study was conducted between October 2022 and May 2023. Data was analyzed using descriptive statistics and paired t-tests with the statistical significance level at 0.05.
The study results revealed that the patient care system for patients with sepsis using the fast-track service system consisted of structural aspects, processes, and outcomes. After implementation, the average knowledge score of nurses increased from 27.25 to 44.75. The percentage of patients entering the fast-track service system increased from 64.86% to 100%. Blood cultures were sent before administering antibiotics, and patients with sepsis received antibiotics within 1 hour. The administration of fluids through the bloodstream at a rate of 30 ml/kg/hr was 100%, and no fatalities were reported. Compliance with the care model was 100%, and staff satisfaction was high. Both knowledge scores and satisfaction significantly increased (p-value < 0.001). The research results indicate that the developed fast-track patient care system can reduce complications and mortality rates.
