The development of practice guidelines for the prevention and control of antimicrobial-resistant bacteria transmission by participation of nursing staff

Abstract

            Participatory Action Research (PAR) was conducted to study the practice guidelines for the prevention and control of antimicrobial-resistant bacteria transmission through the participation of nursing staff using a cyclical process of planning, acting, observing, and reflecting three cycles at Uthai Thani Hospital, focusing on intensive care units, male medical   wards, and female medical wards from October 2023 to February 2024. A purposive sample selection was made with two specific groups :
1) a group of 8 nursing staff members involved in developing practices and 2) 82 nursing staff members who implemented the prevention practices in patient care, along with 431 patients during the research period.

            The research tools included guidelines for group discussions, personal data recording forms for nursing staff, observation recording forms for practice adherence, knowledge assessment forms, and nosocomial infection surveillance forms. Data was analyzed using content analysis, percentages, means, and ranges, with differences compared using t-tests and chi-square statistics.

            After the development phase, the research yielded practice guidelines for prevention and control of antimicrobial-resistant bacteria transmission, wherein staff participated in designing practices consisted of 1) history taking and communication 2) monitoring culture results 3) separating patients
4) wearing personal protective equipment 5) separating patients equipment 6) hand hygiene
7) environmental cleaning 8) providing advice to relatives/caregivers 9) nursing sequence arrangement and 10)  discontinue isolation. After the development, the knowledge scores of nursing staff increased higher than before the development. The compliance rate increased from 64.59% to 95.27%. and the percentage of multidrug-resistant infections under special microbial control in hospitals decreased from 5.14% to 1.38%.
