Objective : To develop and study the results of a self-management model in diabetic patients combined with the use of technology for remote health monitoring (Telehealth).
Study method : Action research from October 2023 - June 2024. Sample group. Including a multidisciplinary team of 12 people, 672 patients with type 2 diabetes. Research tools include self-management models for diabetic patients. Data collection tools include telehealth monitoring technology programs consisting of 1) Application 2) Line Program Zoom meeting and 3) Program HosXP operates in 4 phases: planning, action, observation and reflection. Quantitative data analysis Using descriptive statistics including number, percentage, average, comparing using pai t-test statistics.
Results of the study : It was found that the care model for diabetic patients receiving care through the telemedicine system monitors their health, provides knowledge and advice on appropriate behavior by case manager nurses. It was found that there were 672 diabetic patients. People had an average increase in self-care behavior from high levels of self-care from 2.64 to 4.37. The average blood sugar level (HbA1C) decreased from 8.49 to 7.69. The multidisciplinary team's satisfaction went from moderate to high. Diabetes patients' satisfaction increased from a moderate level to a high level. which increased significantly at the 0.05 level
Conclusion : Model of care for diabetic patients who receive care through this telemedicine system. Patients are encouraged to self-manage, resulting in appropriate health behaviors. and better disease control
