Abstract

Abstract

            The strategy is a tool to set management guidelines in accordance with the environment for maximum efficiency and efficiency. This research aims to assess the effectiveness, result. The problems of the 2023 Maha Sarakham Provincial Health System Strategic Plan and the development guidelines for improving the effectiveness of the 2024 Provincial Health System Strategic Plan, was mixed methods research. The population is (1) the performance of 2023, according to the six strategic indicators of every district health service network. (2) Plan coordinator in the work group of the Provincial Public Health Office and the district health service network, totaling 13 locations. The sample group was determined by purposive random sampling according to criteria. Qualitative data were collected with key informants. Tools were work performance inspection form and interview form. Data analysis by descriptive and inferential statistics. The research found that the total of 6 strategies averaged the evaluation score 4.02 (very good level). The highest evaluation strategy is Strategy 5, People Excellent, 4.98 (very good level), followed by Strategy 3, Health Promotion, Disease Prevention and Consumer Protection Excellent, 4.13 (very good level) Strategy 6, Governance Excellent, evaluation scores 3.99 (good level). Strategy 4 Service Excellent with an average evaluation score of 3.84 (good level). Strategy 1 Strong and sustainable development of self-health management capabilities for people, communities, and average evaluation score 3.17 (good level). Districts with the highest three evaluation scores: (1) Borabue (2) Kudrang (3) Kae Dam, Chiang Yuen, Na Chueak with the evaluation scores of 4.20, 4.16 and 4.14 Which is a very good level.

            Comparing the health outcomes target with the provincial and district LE/HALE values in 2023 as set by the announcement of the target of Maha Sarakham province with the 2023 LE/HALE values of 74.90 years, which did not meet the target. The performance was 0.10 years below the target and the HALE value was 67.87 years of achievement with more than 0.37 years of achievement. Overall, it was found that the LE value reached the target of 5 districts: Nadun, Borabue, Kae Dam, Phayakkhaphumphisai and Kantharawichai. The remaining 7 districts failed to achieve the highest goal, Kantharawichai, with more than the goal 1.61 years, Na Chueak Districts that failed to meet their target of 2.07 years. Overall, 5 districts reached their HALE target of Nadun, Borabue, Kae Dam, Phayakkhaphumpisai and Kantharawichai, while the remaining 7 districts failed to meet their HALE target. Kantaravichai District was able to achieve the highest HALE target with more than the target of 1.11 years. For the district that did not achieve its target the most was Chuen Chom District, performance was 1.96 years below the HALE target.

            The correlation between the performance of the 2023 Maha Sarakham Provincial Health System Strategic Plan and the results of the 2023 Maha Sarakham Provincial Health System Strategic Plan found that the average evaluation score of all strategies does not correlate with the LE and HALE values of the people in Maha Sarakham Province (r= 0.052, p-value= .866 and r= 0.066, p-value= .831) This research discovered six major problems in implementing the 2023 Maha Sarakham Provincial Health System Strategic Plan: (1) The time frame for establishing provincial and district level action plans must be completed within the first quarter of the year. (2) The time frame for preparing the strategic plan and indicators was delayed. This must be completed before the start of that fiscal year. (3) The project implementation period is short for 4 - 6 months. (4) The annual government performance evaluation period is not over 12 months. (5) Some performance indicators that measure performance results have not been achieved in a single year.
(6) The performance indicators must be minimized.  Because it cannot clearly reflect health outcomes and has found seven key ways to improve the effectiveness of the Provincial Health System Strategy in 2024, namely (1) Display of indicators on a quarterly and monthly basis. (2) Presenting progress to the provincial and district management teams once a month. (3) The announcement is a provincial governor's policy on goals and indicators that affect public health outcomes. (4) Reviewing lessons learned from performance indicators that do not meet targets once a quarter. (5) Building morale and performance at the plan level for provincial and CUP level workers. (6) Follow up on progress of workers at the district health service network once a month. (7) Coordination of primary health service outside the Ministry of Public Health with the secondary level intensively and must be more effective.
