The development a discharge planning model for traumatic head injury with surgery patients by multidisciplinary team in neuro surgery intensive care unit of NongKhai Hospital.

Abstract

                                                                                                                 ABSTRACT

           The study isresearch and development study aimed to develop a discharge planning model for traumatic head injury with surgery patients through the collaborative efforts of a multidisciplinary team in neuro surgery department of Nong Khai Hospital. The process of the model’s development includes 4 phase consisted of establishing a multidisciplinary team and determining its staff position, identifying problems concerning discharge planning for traumatic head injury with surgery patients, developed discharge planning model, according to the implementation discharge planning model, evaluation and revisability to determine activities for discharge planning. The developed discharge planning model was tested with groups of fifteen and fifteen traumatic head injury with surgery patients and was then adjusted to a more suitable plan.

           The results of the study were as follows the discharge plan consisted of providing daily treatments and rehabilitation activities, implementing nursing care concerning both the physical and also emotional aspects of the patients, offering education pertinent to drug usage and feeding techniques, and providing continuing home care. By using discharge planning fourteen days. After the developed discharge plan was tested with the patients, the results showed that average hospital stay of the patients was about 15.56 days, the average of medical expense of each patient was 157,245 Baht, The results of the study showed that the mean scores of caregiver's knowledge after the development were statistically higher than scores before the development (p<0.01). The mean scores of caregiver's capabilities after discharge planning were statistically higher than scores before discharge planning (p<0.01). The satisfaction levels of patients and caregiver were highest level and the multidisciplinary team was proud of their work and had a high level of satisfaction with their services,. The results of this study indicated the discharge planning model developed by multidisciplinary team could improve knowledge and caregiver’s capabilities
