Development of Laboratory Quality in Primary Care Units Amid the Situation of Mission Transfer within the Health Service Network of Phayakkhaphumphisai District, Mahasarakham Province.

Abstract

Abstract

            Background : The transfer of missions to local government organization causes changes and affects the development of laboratory quality in primary care unit (PCU), causing a lack of continuity of operations. Therefore, in order to ensure that the people will receive quality laboratory services, the researchers therefore studied the operating model with the objective of ensuring that laboratory quality development activities in PCU continuous and sustainable.

            Materials and Methods : The action research, There are 65 people participating in the research, including medical technologist, PCU staff, and village health volunteers. Conduct research in 2023 Using a 4-phase development cycle PAOR: Plan, Action, Observation and Reflection.

            Results : The transfer of missions to local government organization causes the transfer of laboratory PCU staff, resulting in operational problems. Therefore a new operating model is established divided into 3 parts. 1.) Laboratory in hospital is the host responsible for supervising consultations, personnel training, Instrument calibration, proficiency testing and implementing RLU 2.) PCU is the main responsible person for carrying out activities with emphasis on staff having understanding and expertise. 3.) Village health volunteers are responsible for integrating work with network partners to carry out activities that the host determines for the community. The results showed that PCU staff had increased knowledge. The results of the proficiency test passed every PCU. They can control to use materials consistent with the workload and reducing problems with expired materials. Village health volunteers receive training to be able to test DTX in health stations. And there is a grouping of community health funds to purchase DTX with quality and reasonable prices.

            Conclusions : The new operating model with operations emphasize the importance of each part with different roles. Resulting in cooperation from all parts is a strong network that can carry out continuous and sustainable quality development in laboratory PCU.
