Development of a Motivation Model for Pulmonary Tuberculosis Patients in Collaboration with the Family

Objective : To develop and evaluate a motivational enhancement model for pulmonary tuberculosis (TB) patients with family participation in Mueang District, Ratchaburi Province.
Methods : This research and development study was conducted in three phases from October 2024 to July 2025: 1) situation assessment of TB patient care, 2) development of motivational enhancement guidelines, and 3) evaluation of the guideline implementation. Data were collected using in-depth interviews, focus group discussions, and questionnaires from 34 pairs of TB patients and family caregivers. Data analysis employed content analysis, descriptive statistics and dependent t-test.
Results : The CARE Model was developed to enhance motivation and consisted of four components: C – Consistent Medication Setup, A – Airborne Precaution Training, R – Regular Check-up System, and E – Education Implementation. Implementation of the model showed that the percentage of caregivers with adequate knowledge regarding the disease and patient care increased from 88.63% to 95.49%. Caregivers’ home care skills improved from
a mean score of 55.79 to 66.84 (p < 0.001), while anxiety levels decreased from a mean score of 5.26 to 2.00 (p < 0.001). In addition, 76.4% of patients demonstrated consistent medication adherence.
Conclusion : The motivational enhancement model with family participation was effective in improving awareness, care skills, reducing anxiety, and increasing treatment adherence in pulmonary TB patients.
