ยา linezolid และการเกิดภาวะ serotonin syndrome
บทคัดย่อ
Linezolid เป็นยาต้านจุลชีพที่มีฤทธิ์ในการฆ่าเชื้อแบคทีเรียแกรมบวกดื้อยา เช่น methicillin-resistant Staphylococcus aureus (MRSA) และ vancomycin-resistant enterococci (VRE) และสามารถใช้ในการรักษา nontuberculous mycobacteria (NTM) ได้ เนื่องด้วย linezolid มีคุณสมบัติยับยั้งเอนไซม์ monoamine oxidase (MAO) แบบไม่เฉพาะเจาะจง ดังนั้นเมื่อได้รับยา linezolid กระบวนการเมตาบอลิซึมของสารสื่อประสาท serotonin ที่อาศัยเอนไซม์ MAO จึงเปลี่ยนแปลงไป serotonin syndrome จึงเป็นผลข้างเคียงหนึ่งที่เกิดขึ้นได้จากการใช้ยา linezolid โดยเฉพาะเมื่อมีการใช้ร่วมกับยาอื่นที่มีผลต่อระดับ serotonin เช่น ยาในกลุ่ม selective serotonin reuptake inhibitors (SSRIs) serotonin norepinephrine reuptake inhibitors (SNRIs) และยากลุ่ม opioids โดยยาที่พบรายงานมากได้แก่ citalopram escitalopram และ methadone องค์การอาหารและยา (Food and Drug Administration; FDA) จึงแนะนำให้หลีกเลี่ยงการใช้ยา linezolid ร่วมกับยากลุ่มเหล่านี้ โดยให้มีช่วงการหยุดยาก่อนเริ่มยาอีกชนิด อย่างไรก็ตามในภาวะคุกคามชีวิต ถ้าจำเป็นต้องได้รับยา linezolid ควรเริ่มยาทันที และการหยุดยากลุ่มที่มีผลต่อระดับ serotonin ควรพิจารณาถึงความเสี่ยงการเกิด serotonin syndrome เทียบกับความจำเป็นในการรักษา และภาวะการถอนยาจากการหยุดยาทันที ในกรณีจำเป็นต้องให้ยาร่วมกันหรือไม่สามารถมีช่วงการหยุดยาได้ควรมีการติดตามอาการและอาการแสดงของภาวะ serotonin syndrome อย่างใกล้ชิด ถ้าเกิดภาวะ serotonin syndrome ควรหยุดยาที่สงสัยว่าเป็นสาเหตุ ให้การรักษาแบบประคับประคอง และพิจารณาการให้ cyproheptadine ที่เป็นยาต้านพิษ
