การผ่าตัดรักษามะเร็งชนิดเมลาโนมาของเยื่อเมือกช่องปาก: รายงานผู้ป่วย 1 ราย
บทคัดย่อ
มะเร็งเยื่อเมือกชนิดเมลาโนมา เป็นเนื้องอกที่พบได้น้อยแต่มีลักษณะการลุกลามที่ก้าวร้าว มีอัตราการรอดชีวิตต่ำ และสามารถแพร่กระจายได้อย่างรวดเร็ว พบได้บ่อยที่เพดานแข็งและสันเหงือกของขากรรไกรบน ในทางคลินิกรอยโรคสามารถพบรอยโรคได้ทั้งที่เป็นเม็ดสีเดี่ยวหรือหลายตำแหน่ง หรืออาจเป็นแบบงอกขยาย หรือเป็นรอยโรคไม่มีเม็ดสี ที่บริเวณผิวหนัง เยื่อเมือก ความหนาของเนื้องอกเป็นปัจจัยที่กำหนดวิธีการรักษาและการพยากรณ์โรค ระยะการแพร่กระจายของเนื้องอกทางคลินิกสามารถแบ่งเป็น 3 ระยะ ซึ่งมีแนวทางในการรักษาที่ต่างกันไปตั้งแต่การผ่าตัดอย่างเดียว การผ่าตัดร่วมกับการฉายแสงรังสี การผ่าตัดร่วมกับการฉายแสงรังสีและเคมีบำบัด การรักษาแบบประคับประคอง (palliative) ร่วมกับการฉายแสงรังสีและเคมีบำบัด บทความนี้มีวัตถุประสงค์เพื่อรายงานผู้ป่วยที่เป็นมะเร็งชนิดเมลาโนมาบริเวณเหงือกของขากรรไกรล่าง ได้ทำการรักษาโดยการผ่าตัดร่วมกับการเลาะต่อมน้ำเหลืองบริเวณลำคอร่วมกับการฉายแสงรังสี หลังจากที่ติดตามผลการรักษาเป็นระยะเวลา 2 ปี ผู้ป่วยมีสภาพปกติ ไม่มีการกลับเป็นซ้ำของเนื้องอก และไม่พบภาวะแทรกซ้อนอื่น ๆ
