Outcomes of Drug Used Management during Home Visit in the Mueang Chonburi District

This descriptive research was aimed to investigate and evaluate drug related problems (DRPs) in home visit. Data were collected between 1 June 2017 - 31 December 2017 in Moo 3, Mueang district, Chonburi province. Out of 72 patients with chronic disease, 39 were female (54.2%) and 33 were male (45.8%), mean age 66.18±16.45 years. The most common chronic disease that found was hypertension (54 cases, 75%) , diabetes(25 cases,34.75%) , cardiovascular disease (19 case,26.38%).In the patients with more than one chronic diseases, hypertension and diabetes were mostly found (30 cases, 41.67%). There were 36 cases (50 %)  that didn’t have the caregiver to prepare medication for the patients.In this study, the family pharmacist was responsible for visiting each patient’s home to investigate the cause and solve the problems.The data were collected by interviewing, observing the patients and recording in the interview form, and analyzed by the family pharmacist.The result in this study revealed that most common drug related problem were inadequate drug use  knowledge of the patients  (44 cases, 55.6%), following by patient noncooperation (31 cases, 43.1%) and using medicine without indication  (29 cases, 40.3%). For the medication errors from medication reconciliation reviewing found that 12 cases (26.08%) received incorrect drug, 12 cases (26.08%) received an inadequate quantity of drug and  21 cases (45.65 %) that the patients didn’t adjust to proper dosage that recommended by the doctor. The average frequency of home visit was 2.5±1.29 times. (max 4 times, min 1 time) After problem management, the result showed that there were 54 cases (75%) that cooperated with the recommendation and followed the advice of the pharmacist and 18 cases (25%) that patients still had the problems.The pharmacist solved the problems by recommending the patients to arrange medication as a meal per day by using organizer box to make patients take the medication correctly.
