แนวทางการจัดการทางคลินิกสำหรับภาวะภูมิไวเกินจากแพคลิแทกเซล: บทความปริทัศน์

ภาวะภูมิไวเกิน (hypersensitivity reactions; HSRs) จากแพคลิแทกเซล มีอุบัติการณ์ร้อยละ 5–30 และมักเกิดในรอบการรักษาที่ 1–2 กลไกหลักเป็นแบบ non-IgE-mediated ผ่านกระบวนการ complement activation–related pseudoallergy (CARPA) โดยมีสารช่วยในตำรับ Cremophor EL เป็นปัจจัยกระตุ้นที่สำคัญ บทความปริทัศน์นี้ สรุปพยาธิสรีรวิทยา ลักษณะทางคลินิก การประเมินความรุนแรงตามเกณฑ์ Common Terminology Criteria for Adverse Events (CTCAE) และแนวทางการจัดการในเวชปฏิบัติ ครอบคลุมการให้ยาป้องกันก่อนการรักษา การจัดการภาวะฉุกเฉิน การให้ยาซ้ำ และการทำยาให้ทนต่อยาแบบเร่งด่วน นอกจากนี้ บทความยังสังเคราะห์แนวทางการดูแลผู้ป่วยหลังเกิด HSRs ตั้งแต่การประเมินความรุนแรง การพิจารณาให้ยา re-challenge หรือการเปลี่ยนสูตรการรักษาให้เหมาะสมกับผู้ป่วยแต่ละราย ทั้งนี้ การเลือกแนวทางการรักษาควรพิจารณาร่วมกันจากระดับความรุนแรงของอาการ หลักฐานเชิงประจักษ์ และบริบทด้านทรัพยากรของสถานพยาบาล เพื่อยกระดับความปลอดภัยและความต่อเนื่องในการรักษาผู้ป่วย
