Antibiotic Use Evaluation in Orthopedic Surgery Patients

Abstract

Background: It is now widely practiced receiving antibiotics to prevent surgical wound infection in patients undergoing surgery. There are many patients undergoing orthopedic surgery in Chao Phya Abhai bhubejhr Hospital. However, there are no clear guidelines for the implementation of antibiotic use in such patients.

Objective: To study the suitability and conformity,including finding drug discrepancies in the use of antibiotics in orthopedic surgery patients.

Method: A retrospective descriptive research by collecting data from medical records of 207 patients undergoing surgery during October 1, 2020, to December 31, 2020. Data were analyzed by descriptive statistics, frequency, and percentage.

Results: total of 207 patients consisted of 148 males (71.51%), 60 females (28.50%), with a mean age of 40 years, most of them had no underlying disease, 168 (81.16%), and 100 (48.31%) had dirty wound and 98 (47.74%) had implantation of internal fixation devices. The process of prescribing antibiotics stat before surgery for 70 items, the drugs were appropriate, 69 items ( 98.57%). In the process of prescribing antibiotics during surgery( 206 patients) , 5 patients requiring medication during surgery, found to be inappropriate 20.00% (1 person). The process of prescribing 231 antibiotics after surgery, 228 (98.70 %) was appropriate. The discontinuation of antibiotics within 24 hours in 205 patients,173 (84.43%) were appropriate according to the criteria and 23 (11.28%) was inappropriate. Overall, 70 preoperative antibiotics administration 33 (47.14%) was improper, all of which were administered immediately too close to the time of surgery. The most common drug discrepancies were wrong time administration after surgery (40 drug items, 8.97 %)

Conclusion : The suitability of antibiotic administration immediately before surgery and not stop prescribing antibiotics within 24 hours was the most inconsistent step with the evaluation criteria, respectively. Therefore, finding a practical approach or clear requirements for such procedures in the surgical process is therefore important.
