Effects of Pharmaceutical Care in Patients with Diabetes Mellitus and Hypertension in the Area of Health Promoting Hospital Network, Loei Hospital

Abstract

Background: A common problem in pharmaceutical services was the medication system for patients with diabetes and hypertension. Therefore, pharmaceutical care was set up to follow up drugs related problems by providing pharmaceutical care through home visit.

Objective: To study the results of pharmaceutical care by home visit.

Methodology: The study was a quasi-experimental study in 46 diabetes and hypertension patients whom were providing pharmaceutical care by family pharmacists in the area of Chaiyaphruek Health Promoting Hospital, Muang District, Loei Province from June-November 2022.

Results: Overall, 60.9% of the participants were female, average age of 67.9±8.3 years. Most of them used drugs from doctors or healthcare facilities only, 6.5% of the drug label were lost, and 4.3% of the package were deteriorated. The adverse drug reactions were found in 4 cases (8.7%). After home visit, the mean scores of medication adherence improved from 5.4±2.6 to 7.1±1.6, a statistically significant increase (p-value<0.05). Mean blood glucose (after 8 hours fasting) after home visit decreased from 143.0±39.6 mg/dl to 110.1±22.0 mg/dl, (p<0.05). Mean systolic blood pressure decreased from 143.7±14.7 to 131.6±10.3 mmHg, (p<0.05). Mean diastolic blood pressure decreased from 73.2±11.1 to 67.2±10.5 mmHg, (p<0.05)

Conclusion: As a result of providing pharmaceutical care through home visit, drug-related problems of patients with diabetes mellitus and hypertension were reduced, and some of the problems with the patient's medication were completely resolved that resulted in decreasing the patient's blood sugar and blood pressure levels.
