Abstract

The purpose of research was to study the situation and prescribing problems and drug related problems in patients. Therefore, innovations have been developed for solving drug prescribing problems and evaluating the effectiveness of using innovations in solving drug prescribing problems. In order to set the guidelines for pharmaceutical care in asthma and chronic obstructive pulmonary disease clinic. The samples were 1631 people of asthma and chronic obstructive pulmonary disease patients in the asthma and chronic obstructive pulmonary disease clinic at Kumphawapi Hospital, Udon Thani Province. The study was conducted from October 1, 2017 to September 30, 2019. The study use of innovation inhaler list chart sheet in Kumphawapi Hospital. The researcher collected data from prescriptions. Pharmaceutical care was analyzed from drug related problem of patients and assessed the use of inhalers. Data analysis using frequency and percentage.

Kumphawapi Hospital is a general hospital. An important problem was found a rotation of doctors on a monthly basis. The problem of improper prescribing among a rotation of doctors in the clinic was the overdose and the prescribing not related to the number of appointment days. This often leads to a delay in consulting a doctor with the same prescribing problem. Therefore, pharmacists have created an innovative inhaler list chart sheet for use in Kumphawapi Hospital by attaching it to every medical examination room in the asthma and chronic obstructive pulmonary disease clinic to let doctors know about prescribing medications for accurate and appropriate of medication. In pharmaceutical care pharmacists have a role in solving drug problems in specific patients. Resulting in problems from the patient’s drug use is reduced. Patients can use inhaler accurately. Trends in emergency room admissions hospitalization as an inpatient and the deaths of patients are reduced and can also reduce the cost of hospital medicine costs a lot per year.

From the research results, The innovative use of inhalers in asthma and chronic obstructive pulmonary disease clinics could be integrated into other health care settings. As a guideline for solving problems prescribtion in the future.
