ผลกระทบของนโยบายการกำกับดูแลการใช้ยาต้านจุลชีพอย่างเหมาะสม ในโรงพยาบาลแพร่
บทคัดย่อ
บทนำ: การใช้ยาต้านจุลชีพอย่างไม่เหมาะสมเป็นปัจจัยสำคัญที่ก่อให้เกิดปัญหาเชื้อดื้อยาและเพิ่มภาระค่าใช้จ่ายด้านการรักษาพยาบาล โรงพยาบาลจึงได้พัฒนาระบบประเมินการใช้ยา (Drug Use Evaluation; DUE) เพื่อส่งเสริมการใช้ยาต้านจุลชีพอย่างสมเหตุผลภายใต้แนวคิดของโปรแกรมกำกับการใช้ยาต้านจุลชีพ (Antimicrobial Stewardship Program)
วัตถุประสงค์: เพื่อประเมินผลกระทบของการดำเนินระบบ DUE ต่อปริมาณการใช้ยาต้านจุลชีพและค่าใช้จ่ายด้านยาต้านจุลชีพในโรงพยาบาล
วิธีการศึกษา: การศึกษานี้เป็นการวิจัยเชิงกึ่งทดลอง โดยใช้การวิเคราะห์อนุกรมเวลาแบบ Interrupted Time Series (ITS) ด้วยวิธี segmented regression เพื่อประเมินการเปลี่ยนแปลงแนวโน้มของการใช้ยาต้านจุลชีพก่อนและหลังการดำเนินระบบ DUE ข้อมูลถูกรวบรวมรายเดือนเป็นระยะเวลา 24 เดือน (ก่อนการดำเนินระบบ 12 เดือน และหลังการดำเนินระบบ 12 เดือน) ตัวชี้วัดหลัก ได้แก่ ปริมาณการใช้ยาต้านจุลชีพในหน่วย defined daily dose ต่อ 1,000 bed-days (DDD/1,000 bed-days) และค่าใช้จ่ายยาต้านจุลชีพรายเดือน
ผลการศึกษา: ผลการวิเคราะห์ ITS พบว่า หลังการดำเนินระบบ DUE แนวโน้มการใช้ยาต้านจุลชีพบางชนิดลดลงอย่างมีนัยสำคัญทางสถิติ ได้แก่ meropenem (β₃=−3.47, p<0.001), piperacillin/tazobactam (β₃=−2.50, p=0.002) และ fosfomycin (β₃=−0.98, p<0.001) ในขณะที่ ampicillin/sulbactam มีแนวโน้มการใช้เพิ่มขึ้นหลังการดำเนินระบบ (β₃=0.42, p=0.021) นอกจากนี้ การดำเนินระบบ DUE ยังมีส่วนช่วยลดค่าใช้จ่ายจากการใช้ยาต้านจุลชีพ โดยเฉพาะในยาต้านจุลชีพกลุ่มออกฤทธิ์กว้าง
