Abstract

Heart failure is a chronic disease caused by abnormalities in the heart and blood vessels, commonly observed and increasingly prevalent globally. It has a high mortality rate (10% annually), leading to prolonged hospital stays and reduced quality of life. This results in significant budget and healthcare workforce implications for the Ministry of Public Health. Additionally, patients with this condition often have multiple comorbidities such as diabetes, hypertension, or chronic kidney disease, requiring various medications and complex management. Presently, the care for heart failure patients has improved, allowing comprehensive services to each hospital. Patient involvement and a multidisciplinary team approach, including physicians, nurses, pharmacists, physiotherapists, and nutritionists, facilitates has develop for efficient and goal-oriented care.

 Indicators for showing service quality in our hospital established a specialized Heart Failure Clinic aimed at educating patients about the condition, optimizing medication regimens, balancing nutrition, assessing fitness, and providing proper exercise guidance. The effectiveness of our operations will be demonstrated through improved patient quality of life assessed via Functional Class evaluation, reduced hospitalization rates, and decreased mortality post-specialized clinic visits for heart failure. This will lead to system enhancements and better-coordinated care plans for heart failure patients. Patients will gain knowledge, understanding, and efficient self-management skills to tackle issues related to heart failure effectively
