Abstract

Abstract

           This study aimed to 1) Evaluation effectiveness of health management village, 2) examine success factors in management, 3) define an operational strategy for the next year. This study is action research .This Action research consisted 3 steps and was conduct Step (1) situational analysis and context study of the village or subdistrict, Step 2) Implementation action plan and step 3) reflection and Evaluation. The sample groups were
1) 26 district primary healthcare staff members, 2) 44 provincial Village Health Volunteer Committee members. The development scope was 13 districts of Mahasarakam Province (1,980 villages). Development period was between October 2015 – September 2016. Data collection tools were 1) Health Management Village Evaluation form, 2) Evaluation skill form for health Volunteer skill. The collected data was analyzed using descriptive and inferential statistics for quantitative data and content analysis for qualitative data. Phase 1 study showed that 1) the 2015 assessment showed that only 95.61% of the villages were covered, with 87 villages in four districts. 2) In the 2015 assessment only 94.75% of the villages passed the requirement. The worst-performing district was Kosumpisai District at 79.40% 2) The plan to solve problems and develop work in 2015 had eight proscess : (1) Participatory problem analysis and planning, (2) policy implementation, (3) training teamwork for competency, (4) Assessment skills development, (5) Development of provincial Health Management Village, (6) Development of supporting information technology, (7) three field visit and experience sharing during implementation, (8) Reflection of development result. Later in 2016, three additional activities were added: provincial knowledge exchange, knowledge sharing, and finding best practices at the village level. In Phase 2, it was found that all eight activities were performed in 2015 (100%), and in 2016 the activities were all performed (11 activities, 100%). In Phase 3, village assessment in 2016 showed that 99.29% of the villages passed evaluation (4.79% increase from 2015). The villages could be divided into excellent (1,167 villages, 58.94%), best (205 villages, 10.35%), good (594 villages, 30%), developing (11 villages, 0.56%). On average, post-test Health Management Village evaluation (in 2016) was higher than pre-test score (in 2015) with statistical significance (p-value =0.02). Result of Health Management Village assessment skill development in the staff and health care workers team was that after the 2016 training, the staff was more skilled than before the training with statistical significance (p-value =0.03). The success factors were
(1) participation, (2) strong policy and implementation, (3) continuous governance and field visit, (4) two- ways and informal communication,
(5) application of It to support the work, (6) evaluation, (7) team work had commitment and unity. The strategy for the next year should have
(1) Participatory action, (2) policy implementation, (3) appreciation and motivation  (4) Assessment skills development for the provincial Health Management Village network, (5) Strengthening of provincial Health Management Village network, (6) Development of supporting information technology, (7) field visit and experience sharing during implementation, (8) Reflection of development result, (9)capacity and competency building of health personal and village team leader, (10) morale building. The study on Effectiveness of Health Management Village in Mahasarakam Province resulted based on M-S-K-P-O-R Model for high level and gaining benefit : 1) M: meeting (meeting of village health volunteer organizations), (2) S:Staff (village health volunteer teams and community healthcare leadership), (3) K:Knowledge (knowledge management and community learning stage),
(4) P:Process (Success of the process), (5) O:Output (Success of the activity) and (6) R:Reflect (Review the healthcare management process).
