Methods : Action research, 1 October 2022 – 30 March 2023. The sample group consisted of 25 service providers, including primary care unit officials, village health volunteers, community leaders, Subdistrict Administrative Organizations, network partners, and service users. Purposeful selection of 68 people. Data collection tools. 1. Research questionnaire 2. Blood pressure recording form 3. Satisfaction questionnaire of service team and service users. Conducted in 3 phases 1. preparation 2. implementation 3. evaluation and quantitative data analysis. Use descriptive statistics: number, percentage, mean, standard deviation. Inferential statistics used paired t-tests and qualitative data. Use content analysis
Results : Health station established by the community to support management. increase access And there is a process for monitoring, measuring, monitoring, and evaluating risks. By applying the 3 Doctors concept together with using technology to support increased self-management behavior. As a result, blood pressure control has increased by a percentage providers and service recipients have increased levels of satisfaction.
Conclusion : The effectiveness of the health station resulted in increased blood pressure control. Patients are satisfied Can be applied to care and follow up on other chronic disease groups.
