Abstract

            This action research aims to develop and evaluate a model for monitoring symptoms of psychiatric patients with community participation. Nong Bua Santu Subdistrict Health Promoting Processing period: January 2021 - September 2022 Conduct development by analyzing problem situations Self-care experience of psychiatric patients so that they do not get sick again from reviewing patient care In-depth interview Meeting to prepare and create understanding Return information to the community Present information on care problems in area of responsibility Jointly design operational guidelines Follow up on results lesson learned Every 2 months research tools Mental status assessment form for psychiatric patients Interview format with open-ended questions Interview form Observation form Patient record form Home visit record form for data analysis Quantitative data : Quantity of qualitative data Use content analysis and check the reliability of the information by triangular method

            The results of the study found that Developed format Developed format  together with (1) Adjust the work of mental health and psychiatry By having professional nurses stationed at the Subdistrict Health Promoting Hospital is the main responsible person (2) Develop the potential of professional nurses To undergo training in the field of mental health and psychiatry Short course 3 days (3) Develop the potential of community mental health networks Community mental health at the village level To have knowledge Skills Effective care for psychiatric patients at home (4) Return information to communities with psychiatric patients who frequently relapse Along with a discussion and meeting Brainstorm to jointly design community-based surveillance and stimulate changes in community attitudes to create awareness and realize the importance of caring for psychiatric patients in the community (5) Lessons learned to create a model for caring for psychiatric patients in the community (6) The community is the searcher Screening patients Identify patient problems and needs (7) Monitor and evaluate Use a multi-channel coordination system Improve operational activities Continuous periodic evaluation of operations Evaluation found that (1) The general public has a sense of belonging Has the potential to search and filter Provide assistance to patients
