Abstract

Abstract

           This research is an action research following Kemmis & McTaggart’s concept which aims to develop care model for dependent elderly persons in health service network of Kaedam District. Study duration was from April 2015 to April 2016. The samples were comprised of 6 groups : 1. In-depth interview group 12 persons 2. Focus group discussion 8 persons
3. Brainstorming group 60 persons 4. Caregivers 45 persons
5. Dependent elderly persons 319 persons and 6. Multidisciplinary team member 20 persons. The tools used in Barthel Activities of Daily Living : ADL, WHOQOL-BREF-THAI, Caregiving knowledge questionnaire for, Focus group discussion guide and In-depth interview. Quantitative data were analyzed by percent mean and standard deviation. Qualitative data were organized and used content analysis.

           The results of this study were :

               1. Village health volunteers lacked of knowledge and skill for dependent elderly care. The action of responsible persons was not clear following care model for dependent elderly persons. The related parties analyzed and planned to solve problems by brainstorming. 

               2. Care model for dependent elderly persons in health service network of Kaedam District consisted of 1) Committee in sub-district and district 2) Coordinator / System manager of dependent elderly care in health service network of district 3) Continuing care from hospital to community 4) Responsible persons in each area 5) Dependent elderly care in health service network of Kaedam district manual 6) Care giver trained 70 hours from Development of Health 7) Training to improve Care give once a year 8) Arrange a meeting to share knowledge once a year. After training found that the sample group of Village health volunteers had average about dependent elderly persons (x = 26.02 ,S.D=1.559) higher than before training(x = 22.42, S.D=2.388). (statistical significance level at .01)

               3. Monitoring and evaluation dependent elderly care 319 persons. After finished process, ADL score was higher and quality of life was higher at good level (44%) Going concern helped long term care for elderly persons in sub-district of health service network of Kaedam District, Mahasarakham Province was completed.
