Aseptic Meningitis in Infant with Kawasaki Disease - เยื่อหุ้มสมองอักเสบในผู้ป่วยคาวาซากิ : รายงานผู้ป่วยเด็ก 1 ราย

Abstract

              A 3-month old boy presented with high fever, diarrhea and convulsion. Physical examination and laboratory investigation results showed aseptic meningitis. A few days later this patient had erythematous rash and conjunctivitis. His oral cavity and lips were deeply red and unilateral cervical lymph node enlarged and swollen with erythema of palms and soles. Echocardiography showed abnormal diameter and internal lumen changed of coronary artery. Perivascular brightness was seen, mild pericardial effusion.  Then the boy was diagnosed asKawasakidisease with meningitis and treated with IVIG and ASA.  The clinical symptoms and signs were improved and discharged later. Echocardiography 6th week after discharge from hospital showed normal coronary artery.

              รายงานผู้ป่วยเด็กอายุ 3 เดือน ส่งตัวมาจากโรงพยาบาลชุมชนด้วยเรื่องไข้สูง ถ่ายเหลวและต่อมามีอาการชักเกร็ง ตรวจพบมีอาการของเยื่อหุ้มสมองอักเสบ ผลการตรวจน้ำไขสันหลังเข้าได้กับภาวะเยื่อหุ้มสมองอักเสบที่เพาะเชื้อแบคทีเรียไม่ขึ้นเชื้อ (aseptic meningitis) ต่อมาพบว่าผู้ป่วยมีอาการผื่น ตาแดง ปากแดง ต่อมน้ำเหลืองที่คอโต มือเท้าบวม ผลการตรวจคลื่นเสียงสะท้อนหัวใจความถี่สูง พบความผิดปรกติของหลอดเลือดโคโรนารีของหัวใจ จึงวินิจฉัยว่าเป็นโรคคาวาซากิ (Kawasaki disease) ได้รับการรักษาโดยให้ intravenous immunoglobulin (IVIG) และ acetylsalicylic acid (ASA) ผู้ป่วยมีอาการดีขึ้นสามารถกลับบ้านได้ และเมื่อติดตามอาการเป็นระยะพบว่าความผิดปรกติของหัวใจจากการตรวจคลื่นเสียงสะท้อนหัวใจความถี่สูงที่ประมาณ 6 สัปดาห์พบเส้นเลือดหัวใจกลับมาเป็นปรกติ
