This prospective, guasi-experimental study was designed to study effectiveness of a self care program in poorly controlled HbA1C in DM type 2 patients from April to September 2008. The 4 categories of self care program including diet control, medications, exercises and global heath care were provided at the clinic of Rong Kwang hospital, Phrae. The samples were 22 DM type 2, patients, diagnosed by a doctor with poorly controlled DM type 2 with HbA1C over 6.5 percent. The effectiveness of self care program was evaluated using an interview form approved by experts in diabetes mellitus. The reliability, Cronbach’s alpha coefficient was 0.86, all data was analyzed by using descriptive statistics and paired t-test at 0.05 level of significance. It was found that, self care practice scores showed very well controlled HbA1C level among the DM type 2 patients as well as diet control, medications, and general health care. However self care program scores on exercises was moderate. The difference of the averages of hemoglobin A1C before and after the study was statistically significant (p < 0.05).
