Co-ordination between Community Pharmacy and Community Caring Clinic: the Essential Factors-การนำร้านยาเอกชนเข้าร่วมเครือข่ายเพื่อให้บริการเภสัชกรรมแก่ผู้ป่วยคลินิกชุมชนอบอุ่น: ปัจจัยที่จำเป็น

บทคัดย่อ

           Co-ordination between medical clinic and community pharmacy to serve patients under the national health security scheme is an innovation in Thailand.  This study was aimed to establish and determine key success factors of the co-ordination between medical clinic and community pharmacy as well as to study the structure of the co-ordination in the network.   The study was conducted by interviewing key stakeholders 6 months after the establishment and by documentary search.  Study site was Mahachai community caring clinic and community pharmacy connected with which was located in Nakhon Ratchasima province.  Data was analyzed by content analysis and summarized into main ideas.  It was found that the co-ordination was success because the policy was open, the community required primary care services, the initiators of the co-ordination had mission to introduce a concept of separation between prescribing and dispensing among physician and pharmacist, good personnel relationship among the founders of the clinic, the respect that pharmacist paid to the founder, the open mind of the pharmacist to accept a new model testing and the setting that clinic and pharmacy closely located.  For the structure of co-ordination, the community pharmacy still provided the traditional services to the community with additional services to patients who brought prescription from clinic.  The structure of administration, structure of information flow and structure of payment were all negotiable.  There was no strong commitment document between the parties.

          In conclusion, this study has shown an innovation of coordinating between community pharmacy and community caring clinic: doctor wrote prescription and pharmacist dispended medicine.  Nevertheless, it should be noticed that the co-ordination in this case were very site specific.
