Abstract

This research aims to develop a screening system, analyze patient medication orders, evaluate outcomes for discrepancies from prescribing medications, and the degree of severity of discrepancies from prescribing captured drugs before and after system development and evaluating the results of accepting the consultation of doctors at H.R.H The Princess Mother’s Centenary Celebrations Hospital, Nakhon Ratchasima Province. This research is action research. There are three phases: (1) the pre-system development phase, (2) the first phase of the system development phase without screening steps for drug orders, and (3) the second phase of the system development phase, in which there is a screening process for drug orders and a pharmacist. The data was collected from the drug prescribing information form of the physicians of chronic disease patients who received treatment in the hospital from October 1, 2019, to September 30, 2020, 4,233 people. The data were analyzed using descriptive statistics such as frequency and percentage.

The study results found that before the development of the screening system, the discrepancy rate of the doctor’s prescription was 17.02%. In the two development phases, the system without screening steps and screening procedures and screening pharmacists, the error rates were 30.85% and 52.13%, respectively. Furthermore, most of the severity level was B, the increase when the drug prescription screening system was developed in the pre-development period and the two periods after the system development period, representing 97.34%, 96.55, and 98.98 percent, respectively. Moreover, there were accepting more doctor consultations after developing an order screening system increased from 93.75 percent to 98.98 percent, indicating that screening system development Analyzing drug prescriptions by pharmacists can prevent drug discrepancies before reaching the patient.
