RESULTS OF SELF-MANAGEMENT APPROACH FOR SURVEILLANCE AND CONTROL OF STROKE RISK FACTORS FOR HYPERTENSION PATIENTS AND THE STROKE FAST TRACK SERVICE BY TECHNOLOGY DIGITAL HEALTH AT HEALTH STATION WITH THE COMMUNITY PARTICIPATION AND STRENGTHENED NETWORK AT
Abstract
This action research aims to study Results of Self-Management approach for Surveillance and Control of Stroke Risk Factors for Hypertension Patients and the Stroke Fast Track Service by Technology digital Health at health station with the Community Participation and strengthened network at Mahasarakham province. The method of operation is classified into 4 step: 1) Organizing the Appreciation Influence Control (AIC) process to study a problem-solving plan by using 60 stakeholder samples who participate in the activity;2 planning and setting up the operating organization 3) Organizing the activity for self- management and surveillance and Control of Risk Factors of Stroke for Hypertension and providing the Stroke Fast Track service depending on the plan determined by the community for a period of 6 months. There were 245 samples of hypertension patient participated in the activity; 4) Evaluating performance and clinical outcome. The data were collected by pre and post operation questionnaire. The qualitative data were analyzed through content analysis and the quantitative data were analyzed through statistical analysis, percentage, mean, and standard deviation. The mean was compared by using Paired t-test statistics.
The result of the study showed that after the operation the sample group had better risk factor adjustment than the pre-operation period ,and all factors had higher post-performance mean score from the pre-operation period .Most of them had more behavior at a good level. By comparing the mean result They had knowledge of stroke syndrome at high level Mean = 13.56, SD = 13.42 followed by exercise behavior, food consumption, stress control, smoking, alcohol drinking, and high blood pressure drug usage at high level (x̄ = 3.75, S.D.=0.22). Samples learned more about the Stroke Fast Track service in sub-district at moderate level (x̄ =14.17, S.D.=1.93) The result of referral fast tract for patient stroke under simulation from the village was quickly at the average of 38 minutes.
