Development of Standard Criteria for Non-Communicable Disease Operation Centers, upgrading the quality of service from Community Primary Health Care Centers.

Abstract

This study is a research and development using a mixed-methods approach. The aim was to develop and validate the quality of the standard criteria for Non - Communicable Diseases (NCDs) Operation Centers to enhance the quality of services provided by Community Primary Health Care Centers. The study was conducted in 76 selected areas using simple random sampling techniques, with a total sample size of 1,200 participants. The results showed that most participants strongly agreed with the standard criteria across all components and indicators, with mean scores ranging from 4.36 to 4.46. Structural validity of the criteria was confirmed using Confirmatory Factor Analysis, yielding acceptable fit indices p-value of Chi-square = 0.077, TLI = 0.999, CFI = 0.999, RMSEA = 0.012, and SRMR
= 0.007. Five latent variables were confirmed as key components of the standard criteria: (1) management of the NCD Operation Center, (2) development and allocation of health care providers, (3) provision of NCD prevention and control services, (4) health service support, and (5) evaluation of outcomes. ROC curve analysis was used to determine the cut - off point for classification. The findings revealed sensitivity of 92.31%, specificity of 66.67%, and an overall correct classification rate of 84.21%. The cut - off point was set at 22, and based on proportional thresholds, five performance levels were established: Diamond (22 points or more), Platinum (16 – 21 points), Gold (11 – 15 points), Silver (6 – 10 points), and Bronze (1 – 5 points). Future research should explore long - term health outcomes in relation to the implementation of these standards and encourage Community Primary Health Care Centers to develop self - assessment practices to ensure sustainability and continuous quality improvement.
