บทคัดย่อ
บทนำ: ยาวาร์ฟาริน (Warfarin) เป็นยาต้านการแข็งตัวของเลือดที่ต้องเฝ้าระวังเป็นพิเศษ เนื่องจาก มีดัชนีการรักษาที่แคบ โรงพยาบาลสมเด็จพระยุพราชเด่นชัยพบผู้ป่วยที่มีค่า INR นอกช่วงการรักษาสูงถึงร้อยละ 52.93 แม้จะเคยได้รับความรู้มาก่อน แต่ยังมีพฤติกรรมการดูแลตนเองที่ไม่เหมาะสม จึงมีความจำเป็นต้องพัฒนาโปรแกรมที่ส่งเสริมการจัดการความรู้และดูแลตนเองอย่างต่อเนื่อง
วัตถุประสงค์: เพื่อพัฒนาโปรแกรมการจัดการความรู้ในการดูแลตนเองของผู้ใช้ยาวาร์ฟาริน งานพยาบาลผู้ป่วยนอก เพื่อศึกษาข้อมูลส่วนบุคคลและข้อมูลด้านสุขภาพของผู้ใช้ยาวาร์ฟาริน งานพยาบาลผู้ป่วยนอก นำเสนอประสิทธิผลของโปรแกรมการจัดการความรู้ในการดูแลตนเองต่อการลดภาวะแทรกซ้อนในผู้ป่วยที่ได้รับยาวาร์ฟาริน งานพยาบาลผู้ป่วยนอก และเพื่อนำเสนอนโยบายการดูแลผู้ป่วยโรคหัวใจที่ใช้ยาวาร์ฟาริน อำเภอเด่นชัย จังหวัดแพร่
วิธีการศึกษา: เป็นการศึกษาแบบ Participatory Action Research : PAR ในผู้ป่วยโรคหัวใจและหลอดเลือดที่ได้รับยาวาร์ฟาริน งานพยาบาลผู้ป่วยนอก โรงพยาบาลสมเด็จพระยุพราชเด่นชัย จังหวัดแพร่ จำนวน 30 ราย ระหว่างวันที่ 2 เมษายน–30 กันยายน 2568
ผลการศึกษา: คะแนนความรู้ในการรับประทานอาหารเพิ่มขึ้นอย่างมีนัยสำคัญ (ก่อน: 11.9±1.57, หลัง: 16.4±1.3, ความแตกต่างของค่าเฉลี่ย=4.5, 95%CI=4.11-4.89), คะแนนพฤติกรรมการรับประทานอาหารเพิ่มขึ้นอย่างมีนัยสำคัญ (ก่อน: 28.9±4.24, หลัง: 38.1±2.88, ความแตกต่างของค่าเฉลี่ย=9.2, 95%CI=8.20-10.06), ระดับค่า INR ปกติเพิ่มจากร้อยละ 46.7 เป็น 76.7 และภาวะเลือดออกผิดปกติลดลงจากร้อยละ 53.3 เป็น 23.3 อย่างมีนัยสำคัญทางสถิติ
สรุป: โปรแกรมการจัดการความรู้ของตนเองต่อการลดภาวะแทรกซ้อนในผู้ป่วยที่ได้รับยาวาร์ฟาริน งานพยาบาลผู้ป่วยนอก โรงพยาบาลสมเด็จพระยุพราชเด่นชัย จังหวัดแพร่ ช่วยให้ผู้ป่วยมีความรู้เพิ่มขึ้นและมีพฤติกรรมการรับประทานอาหารที่ดีขึ้น ส่งผลให้ระดับ INR ที่อยู่ในระดับปกติเพิ่มขึ้นและลดภาวะเลือดออกผิดปกติได้
