The Effectiveness of a Self- Knowledge Management Program in reducing complications in Patients Receiving Warfarin at Den Chai Crown Prince Hospital Phrae Province
Abstract
Background: Warfarin, a narrow therapeutic index anticoagulant, requires close monitoring. At Somdej Phra Yupparaj Den Chai Hospital, 52.93% of patients had INR values outside the therapeutic range despite prior education, highlighting the need for a program to improve knowledge management and self-care behaviors.
Objectives: To develop a knowledge management program for self-care among warfarin users attending the outpatient nursing unit, To study personal and health-related data of patients receiving warfarin in the outpatient nursing unit, To present the effectiveness of the self-knowledge management program in reducing complications among patients receiving warfarin in the outpatient nursing unit and To propose care policies for cardiac patients using warfarin in Denchai District, Phrae Province.
Study design: This Participatory Action Research (PAR) was conducted among 30 cardiovascular patients receiving warfarin at the outpatient nursing unit of Somdet Phra Yupparaj Den Chai Hospital, Phrae Province. The study was conducted from April 2 to September 30, 2025.
Results: Knowledge scores regarding dietary intake significantly increased (pre:11.9±1.57, post: 16.4±1.3, mean difference=4.5, 95% CI=4.11–4.89). Dietary behavior scores also significantly improved (pre: 28.9±4.24, post: 38.1±2.88, mean difference=9.2, 95% CI=8.20–10.06). The proportion of patients with normal INR levels increased from 46.7% to 76.7%, was conducted the incidence of abnormal bleeding decreased from 53.3% to 23.3%, both showing statistical significance.
Conclusion: The self-knowledge management program effectively enhanced patients’ knowledge and improved dietary behavior, resulting in a higher proportion of patients with normal INR levels and a lower incidence of abnormal bleeding among warfarin users at Somdet Phra Yupparaj Den Chai Hospital, Phrae Province.
