The Effects of Nursing Practice Guideline for cataract surgery patients at Phrae Hospital

Abstract

Background:   Cataract is a major eye problem in Thailand. It is the most important cause of blindness. Using development of Nursing Practice Guideline for cataract surgery patients in order to provide proper care. It can reduce the complications and hospital readmissions. It also increase the quality of life when they are at home.

Objective:     To study the effect of using Nursing Practice Guideline for cataract surgery patients on hospital readmissions, complications, knowledge, self-care ability and the satisfaction of patients and relatives including registered nurses in the ophthalmology otolaryngology ward, Phrae Hospital.

Study design: Interrupted time design in cataract surgery patients in the ophthalmology otolaryngology ward at Phrae hospital. Patients did not use guideline group, collected data between April 2017 and July 2017 and received usual care. Patients used evidence based guideline group, collected data between August 2017 and November 2017.

Results:         There were 120 patients in this study that divided into the used guideline group and did not use guideline group equally. The used guideline group had more knowledge scores (12.5±0.7, 10.5±1.2, p=<0.001). There was not significant 

difference in overall performance scores such as eye drops, ointment and eye cleaning between the two groups. There were not hospital readmissions and complications in the patients group who used guideline. In the patients group who did not use guideline found 2 cases hospital readmissions (3.3 %), hyphema

in 1 case (1.7 %) and lens dislocation in 1 case (1.7 %). Patients and Caregivers who used guideline group had satisfaction at high level of knowledge and practice skills more than patients did not use guideline group (p < 0.001). Nurses’ opinion about the effectiveness of Nursing Practice Guideline at high level was easy and convenience to use 88.9 %. All of nurse were satisfaction at high level in clarity, appropriated to use, valuable, and satisfaction in Nursing Practice Guideline.

Conclusion:   Evidence-based Nursing Practice Guideline for cataract surgery patients effect on reduce hospital readmissions and complications after surgery. It should be considered to use in clinical practice.
