Effectiveness of Implementing Clinical Practice Guidelines for Pain Management among Surgical Critically Ill Patients in Surgical Intensive Care Unit, Phrae Hospital

Abstract

Background: Pain is a common problem in surgical critically ill patients. Pain is arising from treatment, resuscitation from medical procedures and nursing results in mortality, respirator use time and length of stay can be increased. 
The researchers therefore adopted clinical practice guidelines for pain management in critically ill patients to help improve the quality of care for surgical critically ill patients.

Objective:          To compare difference of pain scare, and to compare satisfaction score between patients with non-using Clinical Practice Guidelines group and using Clinical Practice Guidelines group.

Design:               This research is interrupted time design intervention. The population studied was critically ill patients treated with major surgery who were admitted to the intensive care units and admitted to the intensive care units, Phrae hospital. The study was conducted between February-May 2021 in a sample of 22 patients with non-using clinical practice guidelines and 22 patients with using clinical practice guidelines. Evaluate pain management outcomes and satisfaction score, using the clinical practice guidelines for pain management among surgical critically ill patients. Comparison of scores before and after using the clinical practice guidelines were analyzed by independent t-test.

Result:               Patients with using clinical practice guidelines group had significantly decreased score of pain level ( =2.24) more than patients with non-using clinical practice guidelines group ( =1.53), statistically significant at the level of  0.001 and patients with using clinical practice guidelines group has significantly increased score of satisfaction ( =7.91) more than patients with non-using clinical practice guidelines group ( =5.68), statistically significant at the level of 0.000

Conclusion:     The implementation of clinical practice guidelines for pain management among surgical critically ill patients can achieve good outcomes in the care of surgical critically ill patients.
