The development of Clinical Practice Guildelines for prevention and care of preterm pregnancy in Phrae hospital

Abstract

Background:    Preterm labor and preterm birth have significant impacts on healthcare system and on the health of both mother and newborn. Preterm labor has a main cause of prolong length of stay in mother. It is a leading cause of preterm birth which is the major cause of neonatal mortality and morbidity.  Clinical practice guildelines are a systematically developed statement that informed by a systemic review of evidence which facilitated efficient practice and improved patient outcomes.

Objective:        To examine effect of the development of clinical practice guildelines for revention and care of preterm pregnancy in Phrae hospital.

Study design:  The study was a historical controlled intervention study. The samples were previous preterm women who attention in ANC Unit, Phrae hospital. There were thirty pregnant women who received usual nursing care between March to September 2019. There were three pregnant women received trial out based on the CPGs and implementing the CPGs on the thirty pregnant women between March to September 2020. Compare general data, efficiency on admitted of preterm labor, preterm birth, readmitted within 7 days and APGAR Score in two groups with percentage, mean and standard deviation. Analysis with t-test and exact probability test and wilcoxon rank-sum test.

Result:             The previous preterm women who attention in ANC Unit, phrae hospital consists of 60 pregnants. Experiment and control group are equal. These two groups were not different in age, body mass index, mother’s underlying disease, drugs used and treatment. the data of newborn such as sex, delivery mode body weight and treatment were not different, compare readmitted within 7 days and APGAR Score in two groups were not different (p=0.313, p=0.313) but pregnant woman who admitted with preterm labor and preterm birth were statistically significant (p=0.017, p=0.010).

Conclusions:    Nurse and health care team should apply clinical practice guildeline for prevent preterm labor and preterm birth.
