Roy Adaptation Model Application in Nursing Care for Spine Surgery Patient: A Case Study

Abstract

        Spine surgery represents a critical event that significantly impacts patients both physically and psychologically. This case study applied Roy's Adaptation Model as a theoretical framework to enable nurses to systematically assess and promote coping mechanisms through the four adaptive modes. The objective of this study was to present a nursing care model based on Roy's Adaptation Model for a 71-year-old female patient undergoing spine surgery.      

       Through behavioral assessment and stimuli analysis, ineffective adaptation was identified across three perioperative phases: 1) Pre-anesthetic phase: The patient exhibited deficits in self-concept and role function modes due to a lack of knowledge regarding the surgical process; stimuli were managed by providing proactive information to align the patient's expectations with reality. 2) Intra-anesthetic phase: Physiological challenges included the risk of significant blood loss and complications from prone positioning; the nurse anesthetist functioned as a regulator mechanism to maintain hemodynamic stability and provide vigilant protection against pressure-related injuries. 3) Post-anesthetic phase: Physiological deficits involved pain management and infection risks; nursing interventions focused on removing stimuli while enhancing the cognator mechanism through distraction techniques, alongside promoting the interdependence mode by involving family members in providing emotional support.

       The application of Roy's six-step nursing process enables nurse anesthetists to systematically analyze root-cause stimuli and manage patients' adaptation problems, ultimately resulting in effective adaptation.
