Abstract

This research and development study aimed to investigate conditions of care, model development and efficiency of follow-up home visit of Schizophrenic patients of at-risk groups by family care team in Wang Chin District of Phrae Province. which applied the semi-experimental research method in one group. Pre-test and post-test were implemented in this stage. The samples include 10 of Schizophrenic Patients of at-risk groups who received care service at Wang Chin Hospital in Wang Chin District of Phrae Province. The data was analyzed with descriptive statistics and Pair t-test statistics.

The results showed that;  applied survey research to examine the condition care given to Schizophrenic patients of at-risk had problems in lack of continuity of care and medicine when they returned home or to the community. They did not accept illness and used alcohol or drugs causing exacerbation and aggressive behaviors. Schizophrenic patients at Wang Chin Hospital in Wang Chin Hospital of Phrae Province consisted of before home visit model for Schizophrenic patients, Team development, declaring of objectives of home visit. The home visit process. It consists of preparing a visit recording form. Division of team roles Recording home visit information and after the home visit It is a summary of the results from home visits, analysis of individual patient problems. and planning the next visit. Including data analysis and synthesis. After implementing the home visit model. the scores of self-care by the patients in the experimental group after the experiment was higher than that of before the experiment with statistical significance at 0.50.  As a result, patients and caregivers practice correct
self-management. To prevent relapse and receive continuous consultation from the family doctor team Therefore, Home Visit Model for Schizophrenic Patients At-Risk Groups in the Community by Family Care Team. Wang Chin District, Phrae Province that is appropriate to the local context, helping to increase efficiency in caring for at-risk schizophrenia patients.
