Abstract

A study on the relationship of lifestyle and treatment plan adaptation of diabetic patients in a chronic non-communicable disease clinic. Sri Muang Mai Hospital It is descriptive research. The objective of this study was to study factors affecting blood sugar control in diabetic patients. and to develop a care model for diabetes patients in Si Muang Mai District Ubon Ratchathani Province The sample group used in the study was 400 patients with type 2 diabetes with HbA1c >7mg% and HbA1c<7mg%. And receive services in a chronic non-communicable disease clinic Sri Muang Mai Hospital Si Mueang Mai District Ubon Ratchathani Province during the study From September 1, 2020, to November 30, 2020, for a period of 3 months. The instruments used in the study were the self-care behaviors of diabetic patients questionnaire The statistics used to analyze the data were percentage, mean, and standard deviation. The study results showed that 1. Most of the patients with type 2 diabetes with HbA1c<7 mg% and HbA1c>7 mg% were female, aged 60 years or over, without caregivers, married/married status, primary education. have a career in agriculture Most had blood sugar levels greater than 180 mg/dl treated for diabetes. with oral medication and treated with both oral and injectable drugs Breakfast at 8:00 a.m., lunch at 12:00 p.m. and dinner at 6:00 p.m. lack of medication was found Patients with type 2 diabetes with HbA1c<7 mg% missed the medication because they forgot to take the pill. Patients with type 2 diabetes with HbA1c >7 mg% were absent from medication due to lack of appointments. 2. Health care behaviors of diabetic patients found that positive self-care behaviors in terms of eating side of medication fitness and continue taking care of yourself It is a behavior that is practiced on a regular basis. (x ̅= 4.74, SD.= .73) 3. Assessment of perceived impact/effects of patients with diabetes found that the knowledge of diabetes The majority of subjects with HbA1c<7 mg% and type 2 diabetic patients with HbA1c>7 mg% had moderate diabetes knowledge at 58.75% and 56.50%. As for the perception of the severity of the disease, it was found that the perception of the severity of the disease was at a high level at 36.75 and 39.25%.
