Abstract

Diabetes and hypertension are among the non-communicable chronic diseases, which are health problems. This is important and tends to increase, leading to overcrowding in hospitals. Therefore, it is distributed to patients receiving services at hospitals near their homes. Ranong Province There is a lack of data collection and follow-up on the clinical outcomes of chronic non-communicable disease patients referred to sub-district health promotion hospitals. The objective was to compare clinical outcomes among non-communicable disease patients who received services between groups receiving services at secondary care facilities and groups receiving services at primary health care facilities through an analytical study using retrospective data collection from the electronic database of Ranong Hospital and the Ranong Provincial Health Center database. We searched among patients diagnosed with diabetes and/or hypertension. During the period from 1 October 2020 to 31 July 2022. The study found that the samples consisted of 479 cases, 256 secondary health care recipients, and 223 primary health care recipients. General information included gender, average age, and comorbidities of both groups that were not different. From the comparison of clinical outcomes, it was found that the secondary care service recipients Clinical outcomes in mean blood glucose (HbA1C) and mean blood pressure (SBP) were lower than those in the primary care group. significantly different at .05 level. As a result, patients have better clinical outcomes than with primary care services. However, primary care facilities as a service unit that plays an important role and is close to patients, clinical outcomes should be monitored closely. Including giving more advice on medication use and behavior to patients.
