Effectiveness of Palliative Care Model for Terminal Cancer Patients in Krathumbaan Hospital Community Network, Samutsakorn Province

The aims of this descriptive study were to develop and describe the effect of a palliative care model for terminal cancer patients of Krathumbaan Hospital Community Network, Samutsakorn province. Samples were purposively selected from 21 hospitalized patients with 37 out patients, totally 58 patients with terminal cancer. Study was conducted from January to December 2015. Descriptive statistics were used to analyze this data. The results of this study revealed that the palliative care model was suitable for terminal cancer patients. Krathumbaan Hospital. Palliative Care Model tools in this study were evaluated and managed following PPS score and pain score. Good death criteria and evaluation of patients and caregivers’ satisfaction were used to report informants resulted. After implement the patients care model, we reported that palliative care terminal cancer patients were good death 96%, 26 patients were dead in hospital and 27 patients were dead at home. Only 2 patients do not good death criteria. Pain control were managed by pain guideline, pain being relieved 98.2%. The 98% of patients and caregivers satisfactory toward pallia-tive care service were also at high level. In conclusion this newly developed caring model is useful for supporting patients with terminal cancer and their families by multidisciplinary providers with comprehen-sively and continuously. Therefore, this newly developed caring model should be introduced to care for terminal patients in the other groups.
