Development of Case Management Model for the Care of Person with Asthma at Chiangyuen Hospital, Maha Sarakham Province

Abstract

The objectives of this study were to develop a case management program for asthma patients care and assess satisfaction of nurses and asthma patients as well as the length of stay in hospital and re-admission within seven days of the patients toward the care program. There were 3 groups of study samples: the multidisciplinary care team, 10 nurses and 20 asthma patients. The study tools included a group discussion guideline, a record on number of admission days, a record on readmission within seven days of asthma patients, and satisfaction assessment forms for the multidisciplinary team, the nurses and the patients with reliability of 0.972, 0.813, and 0.879, respectively. Data were analyzed by using descriptive statistics as well as the Mann-Withney U test for patients' satisfaction comparison before and after the intervention and Wilcoxon signed ranks test for the comparision among the multidisciplinary team and nurses. As for the results, a case management program for asthmatic patients was developed in which nurses are case managers working with the multidisciplinary team. Satisfaction assessment revealed significant satisfaction among all 3 groups (the multidisciplinary care team, the nurses and the patients). The duration of admissions was significantly reduced by the program, althouth the rate of readmission within seven days was not different. In conclusion, the asthma case management model could be used to improve the case management of asthma patients.
