บทคัดย่อ

           Large defects of the face or oral cavity may be difficult to reconstruct, and usually impairment of functions and cosmetic occur after operation, Forehead flap is one of the regional flap which play the important role in immediate reconstruction decrease hospitalization, good function and cosmetic. Forehead flap was originally designed in 1816 for reconstruction of the nose, and became popular only in 1961 when its benefits was demonstrated, so itus named "Temporal Flap". It provided large surface area as much as lOO to 200 sq;cm. The result of forehead flap usedin Rajvithi hospital will be discussed.

          ผู้ป่วยที่เป็นโรคมะเร็งทางการศีรษะและคอระยะท้าย ๆ เวลาผ่าตัดจะเกิดร่องรอยความพิการของแผลผ่าตัดใหญ่มากทำให้เกิดปัญหากับการซ่อมแซมและเสริมสร้างอวัยวะที่ถูกตัดออกไปให้มีสภาพและการใช้งานได้คงเดิมซึ่งจะต้องคำนึงถึงความสวยงาม และประสิทธิภาพการใช้งานให้คงเดิมมากที่สุด การใช้ Forehead Flap มาซ่อมแซมและผ่าตัดเป็นการผ่าตัดที่นิยมวิธีหนึ่ง ซึ่งทำให้แผลหายเร็วและเป็น Flap ที่ให้ได้ขนาดใหญ่ถึง 100-200 ตารางเซนติเมตร มีการใช้ Flap นี้ตั้งแต่ปี ค.ศ. 1816 แต่พึงมาได้รับความนิยมมากในปี ค.ศ. 1961 และเรียก Flap นี้ว่า “Temporal flap” รายงานฉบับนี้อธิบายถึงภาวะแทรกซ้อนจากการผ่าตัดที่คาดไม่ถึง ซึ่งได้เกิดกับผู้ป่วยรายหนึ่งของโรงพยาบาลราชวิถี ดังมีในรายละเอียด
