Abstract

The objective of Research and Development was improve nursing service system in Head Injury Patients from Traffic Accidents in Emergency Room and Comparison results before – after develop system. Follow the nursing process. Based on system theory framework. Samplings were two groups : such as patients who were injured in the road accident 60 patients were purposive sampling and professional nurses 30 persons. The data were obtained from medical records of head injuries from road accidents. From the observe practice of professional nurses to care head injury patients in emergency room during September 1, 2016 to September 30, 2017, there are 5 steps: steps 1) Preparation steps 2) Design steps 3) Planning step 4) Implement Steps 5) evaluate summarize results and develop. The research tool had two parts: used to conduct the research and used to collect data were created. The content validity was determined by 5 experts updated for modify as suggested. The reliability with professional nurses, emergency room in Regional hospital of North. Confidence has 0.81. Data were analyzed by descriptive statistics Independent t-test and Pair t-test. Use technique preparation, proceed and empower.

The results were new sustainable nursing services system by participatory. After improve system : As a result, Nurses in emergency room have knowledge to assessment accuracy increased from 70% to 100%, Quickly decided to save life increased from 60% to 100%, Decision support, and reporting doctor in time increased from 63.33% to 100%, Sent sample cases to operate increased from 40% to 80% , Convenient system management makes times to send patients from emergency room to operation room had faster from 25 minutes to 20.17 minutes Effected to Head Injury Patients from traffic accidents have Glasgow coma score (GCS), Glasgow outcome scale (GOS) had increased average scores and decreased length of stay (LOS) significantly (p<0.05). Personnel prepared to use equipment increased from 76% to 100%. The nurses were involved, confidence and satisfaction with the new developed nursing system. At the highest level, 96.67%
