Abstract

Background:  Exacerbation in patients with chronic obstructive pulmonary disease (COPD) is recurring symptom. Tend to rise little by little without dyspnea until respiratory failure. The combination of clinical practice guidelines (CPGs) and the knowledge gained about warning signs can help reduce recurrence and reduce the rate of hospitalization.

Objective:        In order to compare the rate of hospital readmission within 28 days with acute exacerbation. In the sample before and after using the clinical practice guidelines and gaining knowledge about warning signs to prevent exacerbations in patients with chronic obstructive pulmonary disease.

Study design:  This research was quasi-experimental. The subjects were 120 patients chronic obstructive pulmonary disease who were hospitalized at Phrae Hospital with acute exacerbation. The control group uses clinical guidelines to plan the distribution of 60 chronic obstructive pulmonary disease patients by collecting historical data from medical records. And the experimental group selected by purposive sampling 60 patients shared a group of clinical practice guidelines (CPGs) with gaining knowledge about warning signs to prevent exacerbations in patients with chronic obstructive pulmonary disease. Data were analyzed by using the SPSS program by using descriptive statistics consisting of frequency distribution, percentage, mean, and standard deviation. Using inferential statistics, consisting of an independent sample; t-test, chi-square, and The Fisher exact test.

Results:            The control group had a hospital readmission percentage of 6.67. The experimental group had a hospital readmission percentage of 1.67. But when analyzed for statistical data found that the experimental group received different guidelines were no statistically significant differences at 0.05 of hospital readmission within 28 days, shows that the developed guidelines tend reducing opportunities for hospital readmit within 28 days.

Conclusion:      Nurses should have the clinical practice guidelines and gaining knowledge about warning signs to prevent exacerbations in patients with Chronic Obstructive Pulmonary Disease, rcsult in improvement of service quality. In addition, PCT MED can use the results of this study as information for improving clinical practices to prevent exacerbations in patients with Chronic Obstructive Pulmonary Disease in Phrae Hospital.
