บทคัดย่อ

ในปัจจุบันปัญหาเรื่องฟันคุดยังเป็นปัญหาที่พบได้บ่อยในกลุ่มผู้ป่วยที่เข้ารับการรักษาทางทันตกรรม⁽⁶⁾ เหตุผลที่ผู้ป่วยมาขอรับการรักษาอาจพบได้ทั้งกรณีที่ผู้ป่วยมีปัญหาโดยตรงจากฟันคุดเช่นมีอาการปวดฟันจากสภาวะฝาเหงือกอักเสบ (pericoronitis) ฟันผุ บวม เป็นต้น ซึ่งการผ่าตัดฟันกรามคุดล่างซี่ที่สาม ทันตแพทย์ผู้ทำการรักษาต้องซักประวัติ ประเมินสภาพผู้ป่วย ตรวจร่างกาย ตรวจภายในช่องปากประกอบกับภาพรังสี เพื่อประเมินตำแหน่งฟันคุดระดับความยากง่ายวางแผนการรักษาตลอดจนการดูแลผู้ป่วยหลังการผ่าตัด เพื่อให้ผู้ป่วยปลอดภัยจากภาวะแทรกซ้อนที่อาจเกิดขึ้น

บทความนี้นำเสนอกรณีศึกษาผู้ป่วยหญิงไทยอายุ 35 ปี มาพบทันตแพทย์เพื่อรับการผ่าตัดฟันกรามซ้ายซี่ที่สาม (38) ตรวจในช่องปากพบฟันคุดซึ่งขึ้นมาในช่องปากบางส่วนและพบฟันผุลึกซี่ 37,38 ไม่มีอาการปวดใด ๆ ฟันซี่ 38 ฝังในกระดูกบางส่วน ไม่มีการบวมของเหงือกบริเวณดังกล่าว จากภาพรังสีพบ ฟันซี่ 38 มีการเอียงตัวแบบ Horizontal Impaction ผลการรักษาหลังการผ่าตัด พบว่ามีการหายของแผลที่ปกติ
